Document Requirements:

• Company deed, for company registration.
• Company NPWP.
• NIB and business license.
• Company address details.
• Employee data, if registering company employees.
• Personal ID or passport, for individual registration.
• Phone number and email address.

Employee Identity Card:

• ID card or passport of company director.
• ID card or passport of authorized representative.
• ID card or passport of employees to be registered.
• Family card, if required.
• Authorization letter, if handled by a representative.

Employment Data:

• Company name and legal entity details.
• Number of employees.
• Employee names and ID numbers.
• Employee position and employment status.
• Salary or wage data, if required.
• Company contact and correspondence details.

Documents to be Delivered:

• BPJS Health registration proof, if applicable.
• BPJS Employment registration proof, if applicable.
• BPJS account access details, if included.
• Employee BPJS membership data.
• Soft copy of completed registration documents.
